Liability Waiver & Participation Agreement
Please complete your information and sign below. Each adult must sign their own waiver. All children must be added to at least one signed waiver.
I understand that participation in activities at Little Brain Lab involves inherent risks, including but not limited to physical injury, illness, and exposure to communicable diseases, including COVID-19. I knowingly and voluntarily assume all risks, known and unknown, for myself and any minor child under my care. I agree to follow all posted safety rules and play guidelines.
Release, Discharge & Agreement Not to Sue I, for myself and on behalf of any minor participant child, and our respective heirs, representatives, and assigns, hereby release, discharge, and agree not to sue Little Brain Lab, its owners, parent brands, subsidiaries, managers, employees, or agents from any and all claims arising from participation in activities at the facility, including claims related to sickness, injury, or death, whether caused directly or indirectly by such participation.
Indemnification & Hold Harmless I agree to indemnify and hold harmless Little Brain Lab from any and all claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys’ fees) arising out of my or my minor child’s participation in activities at the facility.
Security Cameras I acknowledge that security cameras are in use throughout the facility for safety purposes and may be reviewed in the event of an incident or injury.
Photo & Media Release I grant Little Brain Lab permission to use photographs, video, or audio recordings of me or my minor child taken at the facility for lawful purposes, including marketing and promotional use, without compensation.
Parent or Legal Guardian Consent I certify that I am the parent or legal guardian of the minor child(ren) listed below and have full authority to execute this agreement on their behalf. I confirm that I have read, understand, and voluntarily agree to all terms of this waiver.
This Participation Agreement, Liability Waiver, and Hold Harmless Agreement is ongoing and has no expiration date.
